Ashtanga Yoga Studio Liability Waiver *

I understand that yoga includes physical movements which can result in injury. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. I hereby waive my right to sue Ashtanga Yoga Studio and all of its teachers for any injury, illness, or death if it should occur as a result of yoga practice in our facility or any other place. I understand that the risk of contracting Covid – 19 is present in any public place. I waive my right to sue Ashtanga Yoga Studio if I contract Covid – 19 during a yoga class or event of any kind at Ashtanga Yoga Studio. I agree to follow all precautions and studio policies with regard to social distancing and wearing masks in Ashtanga Yoga Studio. Yoga is not a substitute for medical attention, examination, diagnosis or treatment. Yoga is not recommended and is not safe under certain medical conditions. By signing, I affirm that I am in good health and physical condition to participate in yoga classes at Ashtanga Yoga Studio. If I am pregnant, become pregnant, or if I am post-natal or post-surgery, my signature verifies that I have a licensed medical practitioner’s approval to participate in yoga classes at Ashtanga Yoga Studio. I hereby agree to irrevocably release and waive any and all claims that I have now or may have in the future against Ashtanga Yoga Studio and its Teachers.

I have read and fully understand and agree to the above terms of this Liability Waiver Agreement. I am signing this agreement voluntarily and recognize that my signature serves as complete and unconditional release of all liability to the greatest extent allowed by law in the State of Oklahoma.
